Trapezius botox temporarily reduces the strength of the trapezius muscle when it has thickened through being constantly contracted across the neck and shoulders. It aims to ease tension in the shoulder and neck, and to support a longer-looking neckline where the muscle’s thickening has shortened its appearance. It is known internationally as trap botox and on social media as barbie botox. At my clinic in Nişantaşı, Istanbul, I plan this treatment with the approach that comes from my background in anaesthesiology and pain management.
Why the trapezius thickens
The trapezius runs from the back of the neck out to the shoulders and down to the middle of the back. Its job is to carry and stabilise the shoulder girdle — in other words, it works all day.
The problem starts when it gets no opportunity to rest. Long hours at a desk, the head drifting forward while looking at a screen, the habit of carrying stress in the shoulders, and weight training with poor technique all leave the muscle constantly contracted. Like any muscle in constant use, the trapezius thickens over time.
This has two consequences. Functionally, chronic tension can appear in the shoulder and neck, along with the feeling of hardness commonly described as knots, and accompanying headache. In terms of appearance, the shoulders sit higher, the neck looks shorter and the collarbone line becomes less defined.
Who it suits, and who it does not
I am putting this near the top of the page because it is the most misunderstood part.
I perform this treatment in people where I find marked thickening or chronic spasm in the trapezius at examination. The complaint may be mainly pain and tension, mainly appearance, or both; what decides is the state of the muscle.
I do not perform this treatment in people whose neck and shoulder line is normal and whose muscle has not thickened. When someone comes wanting a longer-looking neckline but I find the muscle normal at examination, I tell them plainly. The trapezius is one of the carrying muscles of the shoulder girdle; there is no case for weakening a muscle that is doing its job for the sake of appearance alone. In that situation we discuss different approaches that would contribute to the appearance.
The procedure
The examination runs differently from a standard aesthetic assessment. I assess the volume and firmness of the muscle by hand, measure the difference between the two sides, and identify the trigger points where spasm is concentrated. I also look at your shoulder range of movement and your posture, because if posture is contributing to the picture, the planning changes.
The treatment is given with fine needles to the identified points. The trapezius is far larger than any facial muscle, so the dosing logic is different; the amount is decided after examination according to your muscle mass and the degree of spasm. It takes around 10 to 15 minutes. Cooling is applied beforehand for comfort.
The effect appears at two different speeds. Relief in the feeling of heaviness and in spasm usually begins within 3 to 7 days. The change in appearance, which depends on the muscle’s volume reducing, becomes noticeable within 3 to 4 weeks. Duration averages 4 to 6 months. I recommend leaving at least 3 months before repeating; in people who continue at regular intervals, the effect is often observed to last longer.
My approach
I do not see this as a standalone aesthetic procedure. Trapezius spasm rarely stands alone; it works together with the muscle chain across the neck, temples and jaw. If teeth grinding is also present, I assess the planning together with masseter botox, because the two areas can feed each other.
Finding a correct posture becomes easier once the muscle relaxes, but lasting improvement in posture requires strengthening the back muscles. For that reason I position this treatment as a starting point rather than a solution in itself. Where appropriate I add complementary approaches such as acupuncture and neural therapy to the plan.
Visiting Istanbul or living here?
A large part of my patients are people living in Istanbul as foreign residents or visiting the city, so the practical questions come up often.
Same-day, no stay required. This is a walk-in treatment. There is no recovery period and you do not need to extend your trip for it.
Flying is not a problem. Cabin pressure does not affect the result. You can travel the same day.
Aftercare
For the best result, a few simple points on the first day are enough:
- Do not massage or rub the treated area.
- Avoid very hot showers for 24 hours, and saunas and steam rooms for the first week.
- Avoid heavy exercise and intense cardio for the first week.
- Avoid alcohol on the day of treatment.
Avoid shoulder and back training in particular during the first week
Avoid carrying heavy bags on the first day.
Mild tenderness may appear at the injection points and settles within a few days. Allergic reactions are rare, which is why treatments are carried out under medical supervision.
What we review together at your consultation
-
Pregnancy and breastfeeding
-
An active skin infection at the treatment site
-
Neuromuscular junction disorders
-
Known hypersensitivity to botulinum toxin components
-
Your current medication and general health
-
A diagnosed shoulder problem or ongoing physiotherapy
-
Your training programme if you do professional weight training
If any of these applies to you, we plan a more suitable alternative rather than proceeding.
Frequently asked questions
Will it stop me using my arms, carrying weight or training?
The treatment is given to identified points to regulate excessive contraction, not to take the muscle out of action. Your daily movements, carrying bags and your training are not restricted. If you train at a professional level, mention it at the examination and I will plan the dose accordingly.
Is it painful?
With very fine needles and cooling beforehand, the treatment is comfortable. Comfort management is professional habit for me as an anaesthesiologist. No separate rest period is needed afterwards; you can return to your day directly.
How much product is used?
The trapezius is a very large muscle compared with facial muscles, so the dosing logic differs. The amount is decided after examination according to your muscle mass, the degree of spasm and your shoulder width. I do not use a standard dose scheme.
I have a cervical disc problem. Can I have this treatment?
In people with a cervical disc problem the trapezius can remain in spasm in order to protect the area, and that spasm becomes a source of tension in its own right. Relaxing the muscle may contribute to that component. However, the treatment is not a treatment for the disc problem itself; if you have that diagnosis, we need to proceed together with the physician following you.
Will it help my headaches?
Where headache accompanies tension in the neck and shoulder muscles, relief in the complaint is reported as the muscle relaxes. This is different from the treatment used for chronic migraine, which covers separate muscle groups and different doses — see the migraine botox page.
Will it correct my posture?
When the muscle that involuntarily pulls the shoulders upwards relaxes, finding a correct posture becomes easier and the shoulders settle naturally. But lasting improvement requires regular exercise strengthening the back muscles; it is better to think of the treatment as the start of that process.
Will my muscles sag or my pain increase when it wears off?
No. As the effect reduces the muscle returns to its normal function; sagging or a change in shape is not expected. Some of my patients describe tension running milder in a muscle that has rested for months, but this varies between individuals.
My neck is already slim — can I have it just to make it look longer?
If your muscle is of normal thickness I do not recommend this treatment. The trapezius carries the shoulder girdle, and if it is doing its job there is no case for weakening it for appearance alone. When I assess the muscle by hand at the examination, I tell you the position plainly.
Will I get the same results I have seen on social media?
It is more accurate to say no. Studies of trapezius treatment have limited participant numbers and narrow long-term data, and the result varies markedly between individuals depending on how much the muscle has thickened. Images shared on social media generally belong to people with marked thickening. At the examination I tell you plainly what a realistic expectation is for you.
Can you tell me the price?
Under the applicable regulations, price information cannot be published on our website. The cost of a treatment depends on the product used, the technique chosen and the extent of the treatment. You can call the clinic for detailed information.
