Sunekos (Aminotherapy)

Sunekos, also known colloquially as the “matrix injection”, is a biorevitalisation product that combines hyaluronic acid with an amino acid cluster (HY6AA) that, according to the manufacturer, is patented. The name “aminotherapy” comes from this: delivering the building blocks of collagen and elastin directly into the dermis. Its target is the skin’s extracellular matrix (ECM); it does not add volume or reshape the face. I work with products in this class that have traceability and a certificate of analysis.

What is aminotherapy and how does it work?

Beneath the wrinkles of ageing skin lies a weakened extracellular matrix: fewer collagen and elastin fibres, less elasticity. The logic of Sunekos is to give fibroblasts the amino acids they need to build these fibres (glycine, L-proline, L-lysine, L-alanine, L-valine, L-leucine) together with HA. In cultured human fibroblasts, proprietary HA–amino acid mixtures were shown to regulate extracellular matrix genes; this was a single-donor laboratory study and is manufacturer-linked ¹. A meta-analysis of amino-acid-enriched HA products reports improvement in wrinkle severity; the authors note that the findings need to be confirmed in larger prospective studies ².

Performa and 1200: two forms

I work with two forms in the Sunekos family. Both contain the same amino acid cluster; the difference is the molecular weight of the HA and the target layer.

The manufacturer recommends Performa for thin-skinned areas, including around the eyes. 1200 is denser and provides support in the deeper layer.

The “cushion” technique. Using both forms in sequence in the same session: 1200 first for deep support, then Performa for the superficial layer. I plan this not for everyone but for skin with marked loss of elasticity; if the problem is superficial, one form is enough and I do not add the second.

Who is it for?

• Thin, “paper-like” skin around the eyes, fine lines, dark circles
• Dull skin that has lost elasticity
• People who do not want volume and want their facial shape unchanged
• Supporting skin quality after botulinum toxin or filler

Which product under the eyes?

Around the eyes the decision depends on the cause: if hollowing dominates, mesotherapy is not enough and filler is discussed; if thin skin, dark circles and mild puffiness dominate, HA–amino acid biorevitalisers or polynucleotide products come forward. The product is chosen at the consultation.

Session plan and expected course

• 2–4 sessions, 3 weeks apart. The effect is cumulative and becomes clearer over the sessions.
• After the course the effect lasts about 6 months; maintenance is a single session.

I decide the number of sessions by the response at the second session, not in advance; if the response is adequate I do not complete four.

Procedure and pain management

The procedure takes 15–20 minutes, by diffuse micro-injection.

Topical anaesthetic cream is standard. Because I am an anaesthesiologist, I treat pain management as a separate part of the procedure. Mild pricking is well tolerated.

Aftercare

First 24 hours

• No make-up on the treated area
• No hot showers, sauna, hammam or steam
• Avoid heavy exercise
• No alcohol
• Do not sleep face-down (facial treatments)
• Do not touch the injection points; do not squeeze or massage the bumps

First 48 hours

• Avoid direct and prolonged sun exposure
• No facial massage, skincare treatments or energy-based device treatments

Expected findings Small bumps (papules) at the injection points lasting 12–24 hours are expected and show that the product has settled in the skin. Mild swelling, tenderness or bruising lasting a few days may occur. Around the eyes, bruising is slightly more likely than on the rest of the face.

Safety and anatomy

Sunekos contains non-cross-linked HA and can be dissolved with hyaluronidase if ever needed. Bumps lasting 12–24 hours, mild swelling and bruising can occur at the injection points; around the eyes the chance of bruising is somewhat higher.

Because allergic reactions can occur rarely, treatments are carried out under medical supervision.

Product and regulatory status: Sunekos forms have been assessed under CE and TİTCK frameworks for their relevant indications. Approval belongs to the product and to a specific indication, not to a technique or to a physician.

Who is not suitable

Treatment is not performed, or is postponed, in the following situations:

• Active infection or inflammation in the treatment area
• Autoimmune disease in an active phase
• Pregnancy and breastfeeding
• Known hypersensitivity to the product components
• Uncontrolled chronic illness
• Untreated bleeding disorder or unregulated anticoagulant use

Without being a medical contraindication, I also consider expectation and treatment to be mismatched in these situations:

• People with under-eye hollowing — mesotherapy does not fill a hollow
• People whose actual need is volume — Sunekos does not add volume, and filler is assessed
• People with advanced tissue laxity who require surgical assessment

Planning your visit

My clinic is in Nişantaşı, Şişli, on the European side of Istanbul, approximately 45 minutes from Istanbul Airport.

For a Sunekos treatment I suggest planning a stay of two to three days: arrival, consultation and treatment, and departure. Because this treatment is planned as two to four sessions three weeks apart, it requires more than one visit. The first session takes place in Istanbul, and we plan the schedule of the following sessions together according to your travel arrangements, with remote follow-up in between. Flying is generally appropriate 24 hours after an injectable treatment.

Consultations are conducted in English. Follow-up after you return home is arranged remotely, and I remain reachable for any questions during the period in which the result develops.
