Mid-Face Filler

Mid-face filler restores fat and bone support lost with age in the front of the cheek, reinforcing the load-bearing structure of the face.

The mid-face is the region between the cheekbones, the under-eyes and the sides of the nose. In a young face it is the fullest part. When it loses volume the face flattens, the under-eye hollow becomes more visible and the nasolabial fold deepens — so the mid-face is less an area in its own right than a platform carrying its neighbours.

Mid-face or cheekbone?

In short: the mid-face addresses the front of the cheek; cheekbone filler addresses the outer line of the face. The mid-face provides volume and a rested expression; the cheekbone provides contour and definition. The full comparison is on the dermal fillers page.

What happens in the mid-face with age

1. Volume loss. Bone density decreases and fat pads shrink.
2. Descent. Tissues move downward, toward the jaw.
3. Result. Flattening in the cheek, a more visible under-eye hollow and a deeper fold beside the nose.

Natural results and volume balance

The aim here is not to enlarge the face forward but to support it upward.

I place product on the bone or in the deep fat layer rather than immediately beneath the skin. This technique aims to prevent the face expanding forward — the overfilled look. Being conservative with quantity is the other factor that determines how natural the result is in this area.

How the treatment works

1. Assessment. Asymmetries and the degree of volume loss are evaluated.
2. Product selection. As a load-bearing area, dense hyaluronic acid products with high lifting capacity are used.
3. Application. A blunt-tip cannula is generally used to reduce tissue trauma. The treatment takes about 20 minutes.

Average longevity is around 12 months ¹².

Safety and anatomy

The main consideration in filler treatment is placing the product in the correct plane relative to the tissue and the vascular anatomy. For that reason I prefer a blunt-tip cannula in suitable areas, working in the deep plane and advancing slowly. In a large published cohort study, the rate of vascular events in experienced hands was reported as roughly one in 6,400 syringes with a needle and one in 40,000 with a cannula — infrequent with either technique, and less frequent still with a cannula ³. Experience also matters: in the same study, rates were markedly lower among physicians with more than five years of practice. Aftercare

A few simple points on the first day are enough to keep the result where we planned it:

• Do not apply make-up or cosmetic products to the treated area for the first 24 hours.
• Do not massage or rub the treated area.
• Avoid very hot showers for the first 24 hours, and saunas and steam rooms for the first week.
• Avoid heavy exercise and intense cardio for the first week.
• Avoid alcohol on the day of treatment, and do not smoke for the first 4 hours.
• Drink plenty of water; hyaluronic acid binds water in the tissue, which helps the result settle.
• Sleep on your back on the first night.
• Avoid pressure on the cheeks for the first 24 hours.

Short-lived mild redness, tenderness or a small bruise may appear at the injection points; these resolve on their own within a few days. Because an allergic reaction can occur rarely, treatments are carried out under medical supervision.

The clinic is in Nişantaşı, Şişli, Istanbul, about 45 minutes from Istanbul Airport. A stay of 2–3 days covers arrival, consultation and treatment, and departure. Injectable fillers need no recovery period; mild swelling may last two to three days, so if you have an event we plan the appointment around it. I do not commit to a plan over messages: we speak before you travel so you know what is realistic, and we confirm it in person on the day. Flying is usually fine 24 hours after an injectable treatment. Remote follow-up and contact are provided, and consultations are held in English.

Frequently asked questions

How does it differ from under-eye filler?

Under-eye filler targets the tear trough directly. Mid-face filler supports the cheek. When the cheek is supported the under-eye appearance improves indirectly, which is why in some patients we reach the intended result without touching the under-eye at all. Which comes first is decided together at consultation.

Does it affect the nasolabial folds?

When the mid-face is supported, cheek tissue is carried upward and the weight resting on the folds beside the nose decreases. This generally produces a noticeable softening of the fold; the aim is to lighten its shadow rather than to remove it entirely. More on the nasolabial fold filler page.
