What Is Exosome IV?

Exosome IV is the administration through a vein of purified extracellular vesicles (exosomes) derived from mesenchymal stem cells. Exosomes are very small vesicles that cells release to carry signals to one another; they contain growth factors, proteins and fragments of RNA. The treatment is cell-free: no living stem cells are given, only the signalling molecules these cells secrete. This is a field whose evidence is still developing; clinical outcome data are limited, and I set expectations accordingly.

Administering drugs and fluids through a vein is the foundation of anaesthesiology training; I practise these treatments within that framework. I do not offer exosomes as a plan on their own. The skin application of exosomes, its mechanism and the product class are explained on the exosome skin therapy page; this page covers only systemic use through a vein. The general framework of IV therapy is on the IV therapy page.

This is where I draw the line with exosomes applied to the skin:

With exosomes, the decision depends on the goal: when surface texture and skin quality are the aim, application to the skin comes to the fore; when general well-being and systemic support are the aim, the intravenous route does. The final choice is made at consultation.

How is it thought to work?

Cells send signals to one another through exosomes; this is called paracrine communication. Exosomes given intravenously are intended to reach the tissues via the circulation and carry these signals. Whether that aim translates into a clinical outcome in humans is a separate question; below I write what the evidence says. This is why I use the language of an intended effect rather than phrases like “it renews your cells”.

When do I consider it?

The situations I assess in my practice: prolonged fatigue and low well-being; recovery after a demanding period or illness; within a healthy-ageing (longevity) approach, together with lifestyle adjustment and a functional medicine assessment; and people who want to support skin health systemically as well as locally. What I observe in these pictures in clinical practice is a reported change in general well-being; I write this as experience, not as a promise. It varies from person to person, and no immediate effect is expected.

Being honest about the level of evidence

That exosomes carry signals between cells and can modulate inflammation has been described in cell and animal studies. There is no large controlled trial showing the effect of systemic intravenous use on fatigue, well-being or ageing in humans; the literature rests on small studies, case series and manufacturer data. A review of registered clinical trials reported that most of them are in lung disease and COVID-19, that the route is mostly intravenous or inhalation, and that there are no standards for product preparation, dosing and reporting 1. Alongside observations reporting improvement there are studies reporting limited benefit; I do not give unsourced percentages. I give exosome IV within this evidence framework, as part of a broader plan and with expectations discussed openly.

The procedure and pain management

Topical anaesthetic cream is standard. Because I am an anaesthesiologist, I treat pain management as a separate part of the procedure.

I plan venous access and the infusion as part of this; what you feel is a brief sting as the line is placed.

Aftercare

Same day

Expected findings A small bruise at the venous access site may last 1–2 days. If redness, warmth or swelling develops at the access site, contact the clinic.

Additional advice

Safety and anatomy

Because allergic reactions can occur rarely, treatments are carried out under medical supervision. Because the product is cell-free, tissue matching is not required.

Product documentation: I obtain the exosome products I use from accredited laboratories that hold a cGMP production permit approved by the Turkish Medicines and Medical Devices Agency (TİTCK) of the Ministry of Health. The laboratories’ operating status can be checked on TİTCK’s manufacturer and distributor search page. Every product I work with comes with its own certificate showing the production process and the analysis results; I work only with products of a purity suitable for intravenous use. Products are transported in the cold chain and used once opened. I do not work with a product that has no documentation. Brand and manufacturer names are not the subject of this page; which product is used is discussed at consultation.

Who is not suitable

Treatment is not performed, or is postponed, in the following situations:

I do not perform these treatments during active cancer treatment. For a person with a history of cancer, I make the decision together with the treating specialist.

In intravenous treatments, with heart or kidney failure the decision is made at consultation, with fluid load in mind.

For a person on immunosuppressive treatment, the decision is made at consultation.

Without being a medical contraindication, I also consider expectation and treatment to be mismatched in these situations:

Suitability is determined by consultation in every case.

Pricing

Under the applicable regulations, price information cannot be published on our website. The cost of a treatment depends on the product used, the technique chosen and the extent of the treatment. You can call the clinic for detailed information.

Frequently asked questions

Is exosome IV a stem cell transplant?

No. The treatment is cell-free; no living stem cells are given, only the purified signalling molecules (exosomes) these cells secrete. This is why tissue matching is not required.

How does exosome IV differ from IV vitamins?

IV vitamins replace a substance that is lacking. With exosomes the aim is not replacement but support for signalling between cells. The two serve different purposes and can be planned in sequence when needed.

When is the effect of exosome IV felt?

No immediate effect is expected. It varies from person to person; some people report a change in general well-being, and some report no difference. I do not give unsourced timeframes.

Is exosome IV enough on its own?

No. I consider it as part of a broader plan, together with sleep, nutrition, stress management and, where needed, a functional medicine assessment.

Planning your visit

My clinic is in Nişantaşı, Şişli, on the European side of Istanbul, approximately 45 minutes from Istanbul Airport.

For exosome IV I suggest planning a stay of two to three days: arrival, consultation and treatment, and departure. Because this treatment may be planned as more than one session, it may require more than one visit. The first session takes place in Istanbul, and we plan the schedule of any following sessions together according to your travel arrangements, with remote follow-up in between. Flying is generally appropriate 24 hours after an injectable treatment.

Consultations are conducted in English. Follow-up after you return home is arranged remotely, and I remain reachable for any questions during the period in which the result develops.

You can read how I offer this treatment at my clinic, IV Point, on the exosome IV therapy page.
